Dear Sir/Madam,
I, [EMPLOYEE NAME], Civil ID [CIVIL ID NUMBER], worked for you as [JOB TITLE] from [START DATE] to [END DATE]. Please pay my end-of-service dues and give me a written statement showing my last monthly wage, length of service, how the indemnity was calculated, the leave balance pay and any other amounts paid or deducted.
By my own calculation with the attached rates, the estimated indemnity is KWD [AMOUNT]. Please explain any difference.
Yours faithfully,

Before you rely on it
Ask your employer for a written final settlement statement and compare it with this calculation.
Check the rates in Article 51 of Law No. 6 of 2010 (as amended) with the Public Authority for Manpower.
Confirm which wage is used: basic only or with allowances — as in your contract and payslips.
Keep copies of your contract, payslips and your resignation or termination letter.
Kuwaiti nationals covered by social insurance may have different rules: ask PIFSS.
