Re: Notice of termination of employment (resignation)
Dear Sir or Madam,
In accordance with section 67 of the Labour Code, I hereby give notice of termination of my employment in the position of [JOB TITLE], which I hold under the employment contract dated [DATE OF EMPLOYMENT CONTRACT].
The notice period is [LENGTH OF NOTICE — ONE OR TWO MONTHS] (section 62 of the Labour Code). It starts on the first day of the calendar month following delivery of this notice and ends on the last day of the relevant calendar month; my employment will therefore end on [LAST DAY OF EMPLOYMENT].
Please settle my final pay, including pay in lieu of any annual leave not taken (section 116 of the Labour Code), and issue my confirmation of employment (potvrdenie o zamestnaní) when my employment ends (section 75(2) of the Labour Code).
Before the end of the notice period I will hand over my work, equipment and access rights as you instruct. Thank you for the cooperation and the trust you have placed in me.
Yours faithfully,

Before you hand in your notice
The notice must be in writing and delivered, otherwise it is invalid (section 61(1)). You do not have to give a reason (section 67).
The notice period is at least one month; if your employment has lasted at least a year on the day of delivery, at least two months (section 62(2) and (6)).
The notice period starts on the first day of the month after delivery and ends on the last day of a month (section 62(7)). Notice delivered on 31 October starts running on 1 November.
Hand over two copies in person and have receipt confirmed with a date and signature on your copy.
A delivered notice can be withdrawn only with the employer’s consent, in writing (section 61(4)).
When you leave, the employer must issue a confirmation of employment (section 75(2)) and a work reference within 15 days of your request (section 75(1)). Untaken basic leave is paid out if you could not take it because your employment ended (section 116(3)).
Alternative: termination by agreement (section 60)
Want to leave earlier? Propose an agreement: employment then ends on the agreed day. The agreement must be in writing (section 60).
“The employer [EMPLOYER], company ID [IČO], and the employee [FULL NAME], born [DATE OF BIRTH], agree under section 60 of the Labour Code that the employment established by the employment contract dated [DATE OF EMPLOYMENT CONTRACT] ends on [END DATE]. This agreement is made in two copies, one for each party.”
If you ask for it, the reasons for ending the employment must be stated in the agreement (section 60(2)); for organisational reasons this matters for severance pay.
